Major transitions like moving out of a childhood home, starting university and getting a job are not easy for anyone. It’s significantly harder, though, for people suffering from disabilities and mental illness. It can take a lot of work to become independent to begin with, and if their lifestyle has a major change, that independence could be set back.
Our service provides assistance with life stage transitions and can reduce the physical and emotional toll on NDIS participants. We help them integrate into their new environment as smoothly as possible by providing care and support in a variety of ways.
One way we provide assistance is by helping participants to build relationships in their new communities. We also help instil self-care plans so they can become independent for the long term and maintain good mental and physical health. It’s our goal to make sure all participants feel empowered to be self-reliant in their new environment so they can become independent, meaningful members of society.
